• Please take time to fill in all applicable patients information, this will ensure a speedy delivery of your spectacles.
• Please make sure that the testing optician has signed and entered thier registration number and stated the PCT area that your GOS1 form is to be submitted.

Valid on orders made on or after1st April 2008

A:  £35.50  B:  £54.00 C: £79.00 D: £178.40 E: £61.40 F: £78.10 G: £101.20 H: £196.10

A+A: Two pairs at £35.50 each

• Please note that accessories cannot be purchased with or with the help of vouchers.

If you have not been offered a NHS spectacle voucher and think that you should be entitled to one this is what to do

You can obtain a form HC1 from your local opticians, or your local DSS office. This form explains the process of applying for financial help towards spectacles and sight examinations.

To use your NHS Spectacle voucher you must select pay by post at the payment page, from there you will be able to exchange your voucher as full or part payment for your spectacles, Just follow the on screen instructions
